FemStrix does not use analytics, advertising pixels, behavioural profiling, or browser-storage tracking on this site. It does not set optional cookies. Hosting and security systems may still process connection information, including an IP address, to deliver and protect the site. That infrastructure processing is separate from the research response and is not deliberately added to it.

The survey asks about age, background, location, work, periods, reproductive health, body measurements, lifestyle and clothing context, medical history, medications, symptoms, and family history. These are sensitive personal information. The research record excludes direct identifiers such as your name, email address, and patient number. FemStrix protects the record as sensitive personal information.

Before the first question, you must make an unchecked age confirmation and a separate unchecked express-consent choice. The notice explains the survey's purpose and asks permission to collect, use, and store survey answers to develop, train, test, and improve FemStrix's PCOS/PMOS pattern-recognition model. FemStrix excludes your name, email address, patient number, and connection IP address from the research record.

Taking part is voluntary. You may decline and still use the public website. If you begin, you must answer every question shown to submit the survey. You may stop before submission. The early-access reward code is only a benefit for using the future FemStrix app. It is not linked to your survey answers and cannot locate or open a research record.

This survey is only for people aged 18 or older. The entry gate asks each participant to confirm that they are at least 18. FemStrix does not collect survey responses from minors through this survey.

Sending a copy to a doctor is optional and is not required to complete the survey. If you choose doctor delivery, a separate express-consent step explains the delivery before asking for identifying details. Those details are kept separate and are not used for research or model training. FemStrix removes them from its delivery systems after completion according to the applicable retention rules. The delivery provider and receiving doctor or clinic may retain records when required.

The survey uses answer-based branching to show relevant questions. Choosing the express-consent box and selecting the final submit button activates collection. After the active research store confirms storage, the answers are collected for and may be used to develop, train, test, and improve the group-level pattern-recognition model described above. The survey does not run that model on a participant, create an individual health score, diagnose a condition, or make an automated decision about that person.

After a successful submission, FemStrix stores the answers in a protected research record for the consented PCOS/PMOS model-development purpose. The record excludes direct identifiers and is kept separate from contact information and optional doctor-delivery details.

Access is limited to authorized people and service providers who need the information for that purpose. FemStrix applies safeguards appropriate to the sensitivity of the information, including access controls and encryption in storage and transit where applicable.

Research records are retained only while needed for the consented model-development purpose and applicable legal obligations. Once that purpose is complete, FemStrix securely destroys the records or, where Québec law permits retention for a serious and legitimate purpose, completes the legally required anonymization process.

The survey is used to develop, train, test, and improve FemStrix's PCOS/PMOS pattern-recognition model. It does not assess, diagnose, or provide medical advice. It does not make a decision about a participant.

Authorized FemStrix personnel and approved service providers may use the survey record only to develop, train, test, and improve the PCOS/PMOS pattern-recognition model and to operate and protect the research systems. It is not used for advertising or marketing.

Service providers receive only the information needed for their approved role and must handle it under applicable contractual, security, and retention requirements. FemStrix assesses applicable privacy requirements before information is processed outside Québec.

Doctor delivery is a separate disclosure made only at your request and with separate express consent. Delivery identifiers are kept out of the research and training record and are removed from FemStrix delivery systems after completion according to the applicable retention rules. The delivery provider and receiving doctor or clinic may keep required records under their own professional, clinical, contractual, or legal duties.

FemStrix must securely destroy personal information when its approved retention period ends, unless the law requires continued retention or the information is anonymized through Québec's legal process for a serious and legitimate purpose.

You may ask to see the personal information FemStrix holds about you. You may also ask FemStrix to correct information that is wrong, incomplete, unclear, or handled without legal authority. You may ask for deletion where the law requires it, including when information was collected or retained unlawfully or must be destroyed because its purpose is complete. Where the law applies, you may ask for a portable copy of eligible digital information or withdraw consent for future use or sharing. You may also ask FemStrix to stop publishing information or remove links to it from search results. Some legal limits apply, and withdrawing consent does not undo earlier authorized actions.

FemStrix must respond in writing promptly and no later than 30 days after receiving your written message. If it is not sufficiently precise, the person responsible for privacy must help identify the information sought. Identity verification may be required before personal information is disclosed. Because survey records exclude direct identifiers, FemStrix may not be able to connect a particular record to a person. The privacy contact will explain the secure steps available and any limits that apply. Do not send survey answers or other medical details by ordinary email. The public early-access code cannot be used to find a response.

Withdrawal applies to future use after FemStrix processes the request. It may not undo model training or other authorized actions completed before the withdrawal was received.

To exercise a right, email the privacy contact and describe the information and right involved while sharing only the minimum contact details needed for a response. You may also contact the Commission d’accès à l’information du Québec.

The updated date will change when this policy changes. If a new purpose requires consent, FemStrix will explain it before processing and ask for a new choice. Silence or continued browsing will not count as express consent to use sensitive information. Material changes must also be announced through a clear website notice before they take effect.
