Survey purpose and terms

• Purpose: With your express consent, FemStrix saves the answers to develop, train, test, and improve its model for finding group-level PCOS/PMOS patterns across background, work, lifestyle, medical history, and symptoms.
• Participation: taking part is your choice. If you begin, you must answer every question shown to submit the survey and receive the code. You may stop and clear your unsent answers at any time before submission.
• Not medical care: this survey does not assess or diagnose you. It does not replace advice from a qualified healthcare professional.
• Privacy: answers are stored without your name, email, or other direct identifiers. They still include sensitive health information and are protected accordingly.
• Why it matters: With informed consent, answers from many people can help researchers study patterns and design more representative women's-health tools. Participation does not guarantee a scientific or medical result.
• Age: you must be 18 years of age or older to participate.
• Doctor delivery: after a successful survey, you may optionally request that a copy be sent to your doctor when a secure delivery service is available. This requires separate consent. Delivery details are not added to the research or training dataset.
• Thank-you code: everyone who completes the survey receives the same shared early-access code. It is a reward only and is not linked to, generated from, or used to identify survey answers.
